Our compassionate certification center provides comprehensive medical marijuana evaluations and certifies qualifying patients. Patients will be seen by a state-approved medical practitioner who is registered with the Connecticut Medical Marijuana Program.

CT Medical Marijuana certification lasts a year, and costs $125 for recertification.

Approved patients will be certified with the Connecticut Medical Marijuana Registry. Once your certification is renewed, the patient may continue to the online registration process necessary to obtain a Medical Marijuana Certificate, which allows the patient to purchase medical marijuana from a licensed Connecticut dispensing facility.

Renewal Requirements

  1. Must have an existing MMJ certification and card.
  2. Must be a legal resident of Connecticut with valid proof of residence, such as a driver’s license or state identification card.
  3. Must have a qualifying medical condition as outlined by the Connecticut Medical Marijuana Program.
  4. Must have medical records showing your current medical diagnosis. You may send your records prior to your confirmed appointment via email to info@connecticutcannabis.net
What do I need to do to renew my card?
All patients must be recertified by their physician every year prior to the expiration date, in order to maintain a valid registration. To avoid a lapse in registration, a patient must be recertified by a physician, access their Biznet account and provide this office with the required documentation and fee.
What is the process to change my caregiver?
A qualifying patient may change primary caregivers at the time of their registration renewal by registering a different primary caregiver.
What is the process to be recertified as a patient?
A patient’s certification expires one year from the date the physician certified the patient through our online registration system. Our service will confirm you (the patient) is eligible to continue using medical marijuana, based on the same criteria you were certified for your initial registration.

Teresa Healy, APRN - CT Medical Marijuana CardsTeresa Healy, APRN, is Board Certified in Family Practice and has been in practice for over 20 years. As a family practitioner, Teresa Healy is experienced in the diagnosis and treatment of a diverse number of medical conditions.

Due to the exceptional safety profile of medical cannabis and low-risk of abuse or dependency, she strongly believes in its naturally therapeutic effects in treating (or at least alleviating) a broad range of ailments and illnesses.

Teresa will take the time to understand your concerns and answer all your questions regarding the cannabis program. She is a member of the American Cannabis Nurses Association and the Association of Cannabis Specialists.

Kelly Tobin is a Board Certified Psychiatric-Mental Health Nurse Practitioner who underwent extensive training working with patients with psychiatric disorders across the lifespan and across multiple practice settings. Her education includes a Master of Science in Nursing, from the University of Saint Joseph in West Hartford, Connecticut and Bachelor of Science in Nursing from the University of Rhode Island in Kingston, RI. She also has more than a decade of experience working as a Registered Nurse in a Level 1 Trauma Center in Hartford, Connecticut.

Kelly has worked with an extremely diverse adult population as a staff nurse. She values the practice of medicine and nursing, however, she also strongly believes in holistic, complimentary, and alternative therapies as treatment options for   patients and is passionate about the palliative effects of marijuana when used for medicinal purposes.
